LIST OF REQUIREMENTS

1. Membership Status Verification Slip (MSVS for Pag-ibig members) original & 2 photocopies.

2. Certification of Employment & Compensation issued by the employer under oath (original & duplicate).

3. Latest Income Tax Return with W-2.

4. Latest Payroll/Pay Slip (certified copy).

5. 3 copies 1x1 picture with signature at the back (husband & wife if applicable).

6. TIN Card/Community Tax Certificate (photocopy only).

7. Birth Certificate/Marriage Contract (if applicable, photocopy only).

8. Proof of Billing Address (Davao Light or DCWD Bill).

9. Special Power of Attorney (HDMF Format).

10. Valid Job Contract for OCW.

11. Audited Financial Statement with CPA report, Business Permit, Cert. of Registration, ITR w/Receipts (for Self-Employed)

12 .CSA/ATD or issuance of PDC with Bank Statement or Bank Certificate.
